This document gives a full description of the study. Second, a consent form will be sent to your referring doctor for you to sign together.

Taking part in this project will have no impact on your medical care or that of the person you represent. We will only use the health data collected during the follow-up: this will not change the care, treatment or number of consultations planned as part of your usual care.
